Dental Service Payment Requirements
Patients are expected to pay (cash, check, Mastercard, Visa or Discover) for all dental services on the day those services are initiated.
If the patient has dental insurance the school may accept assignment or complete forms for the patient. If we accept assignment, a pretreatment estimate must be obtained from the carrier before treatment is initiated, and the patient will be required to pay their full portion (actual or estimated) up-front.
Patients with Medicaid must show their Medicaid card at each visit. If services are planned that require prior authorization the request must be sent by the clerk, and approval obtained before the initiation of such treatment(s). If services are not covered or the patient is found to be ineligible for Medicaid or has not met any spend down requirements, the patient will be required to pay for such services on the day it is provided.
Some patients may have another party who will pay for their dental care. We require written authorization from this other third party for each procedure.
Patients requiring extensive care in excess of $1000 and who express financial difficulty may be considered for a contract payment method. No services will be started until the contract has been signed and a down payment made.
DENTAL FACULTY PRIVATE PRACTICE (DFPP)
The Dental Faculty Private Practice extends an invitation to patients desiring to receive quality oral health care in a conveniently located private-practice setting.
The Dental Faculty Private Practice consists of Full-Time Faculty Members who teach and work at the Indiana University School of Dentistry. It is a general dentistry and multi-specialty office that is conveniently located downtown in Indianapolis at the Indiana University School of Dentistry.
Who is eligible to be a DFPP patient?
Anyone with an interest in being cared for as a private practice patient by an IU faculty member is welcome. Persons of all ages are seen in our facility, which is fully equipped, contemporary, and designed to enhance patient privacy. IUPUI students and employees find DFPP an especially convenient place to receive oral health care---they bring their families too! Since the dental school is just a few blocks from Monument Circle, the DFPP facility is easily accessible to downtown residents and office workers. But our patients also come to us from throughout the city and from many other communities in central Indiana and the Midwest.
Is being a DFPP patient different from being a dental school patient?
Yes. In the dental school's other clinics, patients are cared for by supervised students who are in training to become dentists or other types of dental professionals. DFPP group members are dentists who hold full-time positions on the IU faculty, and they conduct their individual practices in a manner similar to other private practices in the community.
How do I become a DFPP patient?
For an appointment or for further information about becoming a patient in the DFPP facility, phone (317) 274-5628. Hours are Monday through Friday from 8 am until 5 pm. Some Saturday appointments are also available. Patients are seen by appointment only. NO REFERRAL IS NECESSARY.
What are the fees for DFPP services?
Fees are consistent with community standards for private practice patients, and payment is expected on the day of service. Services can be paid for by cash, personal check, Visa or MasterCard. We will file claims for reimbursement to patients who have dental insurance.
How do I find the DFPP Clinic?
We're easy to find! The dental school's address is 1121 W. Michigan Street, Indianapolis. We are in Room 285 on the second floor. The main lobby receptionists will be happy to direct you to our clinic. They are on the first floor near the patient entrance.
