Dr. Russell has been recommending Xylitol Chewing Gum for his Greene County, Ohio, patients since the anti-decay effect was discovered.

DocXylitol (an alter-ego of Dr. Russell at Yellow Springs Dental Care) feels the Best Dentistry is Preventive Dentistry. 6 or 8 Chiclets a day will help keep the Dentist away

You've seen the claims on sugarless gum wrappers. Packages of Orbit say the chewing gum helps strengthen the teeth. And a package of Trident gum containing xylitol says it helps prevent cavities. So are these claims solid?

"You know, it depends on how much of the xylitol is in that product," says Francisco Ramos-Gomez, a pediatric dentist at U.C.L.A. "And how many pieces you chew per day."

DocXylitol says the popularity of sugar-free gums has been one step in the improvement of children's oral health. Since consuming lots of sucrose — or table sugar — can lead to cavities, there's a benefit simply in taking it out of gum.

One Step Further

Gum makers have gone one step further by adding ingredients such as the natural sweetener xylitol. That means there's potentially even more of a benefit.

"Xylitol is an anti-microbial — so it's acting against the bacteria themselves," says Peter Milgrom, a dentist at the University of Washington. "So it helps prevent tooth decay."

It's important to note that studies suggest that sugarless gums made of another sugar substitute, called sorbitol, do not seem to have the same preventive effect.

Milgrom and his colleagues have studied how much xylitol is needed to actually knock out bacteria and prevent cavities. And he has two pieces of advice. First: Read the ingredient list on the gum package. If xylitol is the first ingredient, then there's probably enough of it to be effective. And second: Chew a lot of it.

"You have to chew at least two pieces, three times a day to have an effect," says Milgrom.

This may sound like an inordinate amount of chewing, but in Finland kids have really gotten used to it. The Finns have been chewing xylitol gum for years as part of a public health initiative to reduce cavities. And Finnish researchers have documented a preventive effect in numerous studies going back to the early 1970s at the University of Turku. Throughout Scandinavia, xylitol products are widely available, from lozenges and toothpastes to chocolate and candy gummy bears, says Ramos-Gomez.

Too Young To Chew Gum?

For kids who are too young to chew gum or for families that don't have access to good preventive care, there may be another option: daily doses of a xylitol syrup. Milgrom has just published the results of a recent experiment with children in the Marshall Islands who were given a strawberry-flavored syrup for about 10 months.

"The mothers just broke off the end of the squishy plastic tube and squirted it in their child's teeth," explains Milgrom. His team documented a 50 to 70 percent reduction in the development of cavities. The children who were given the highest doses of the syrup saw the greatest benefit.

Pediatric dentists say it's important to remember that xylitol is not a magic bullet in fighting tooth decay. Parents need to stay on top of all the other known prevention methods.

"Control of the diet, regular use of fluoride toothpaste and good ongoing supervision by a dentist are all important," says Burton Edelstein of the College of Dental Medicine at Columbia University. And, of course, so are daily nudges to remind your kids to brush.

There's also indirect evidence to support the claim that gums that have sorbitol, like Orbit, can help fight cavities by strengthening teeth, says Edelstein. It's a claim accepted by the American Dental Association.

"Sorbitol can increase saliva flow, which secondarily strengthens teeth," explains Edelstein. And at the same time, sorbitol is not sugar, so it doesn't cause the bacteria to produce that acid than can lead to cavities. But Edelstein says he's not aware of any studies that directly show that sorbitol-based chewing gums can directly strengthen the teeth.

The state won special attention for its efforts to provide dental sealants, which prevent cavities. We Teach CAVITY PREVENTION.

Xenia and Fairborn kids can have fewer dental problems with Preventive Dentistry.

All states have room for improvement, according to the report, which highlights the importance of preventive care to drive down the incidence and cost of more expensive treatments later.

The report estimates that 17million children, or one in five, go without the dental care they need every year.

"Unlike many dilemmas in health care ... this one is one that can be fixed," said Shelly Gehshan, director of the Pew Children's Dental Campaign.

Ohio received kudos for meeting five of eight benchmarks set by the group. The six states that met six or more earned A's.

One of the benchmarks Ohio did not meet - and the one many dentists see as the main obstacle to better access to care for poor children - involves the money they are paid for caring for Medicaid-eligible children.

On average, dentists in the United States are paid about 61 percent of their median retail fees for treating young Medicaid patients. In Ohio, they are paid 48 percent.

"If we're going to get Ohio to where it needs to be with regard to oral health, it is going to be by fixing the Medicaid program," said David Owsiany, executive director of the Ohio Dental Association.

Ohio didn't meet the Pew benchmarks in two other areas. One looked at whether dental hygienists could see children in the dental-sealant program without a prior exam by a dentist; the other looked at whether the state has added new categories of primary dental providers.

The dental association is in favor of pending legislation that would allow hygienists to perform preventive services with less supervision by dentists.

The Ohio Department of Health deserves much of the credit for efforts including the sealant program, and the state is doing well given the size of its population, he said.

Almost 40 percent of preschool kids have tooth decay. Greene County Ohio kids need early visits to the dentist.

It's the main reason so many of us feel such trepidation when faced with a trip to the dentist.

But the dreaded drill could soon be a thing of the past thanks to a new technique in which teeth are treated with acid gel squirted from a syringe.

Although acid in food and drink is one of the biggest causes of decay, scientists say its corrosive properties become an advantage when it comes to removing rotting parts of teeth.

And while a drill can destroy healthy parts of the tooth, the makers of the pencil-sized 'Icon' syringe say their treatment is far more efficient at removing only the diseased area - and less traumatic.

First, a rubber 'collar' is placed around the target tooth to protect its neighbours. The four-inch syringe then applies a spot of the gel to the discoloured and decaying part.

Within a couple of minutes the acid etches through the enamel into the cavity below, which is then cleaned then dried using ethanol. FInally, the dentist injects a quick-drying resin into the hole which hardens quickly under a high-energy blue light.

The finished filling looks like normal tooth enamel and the whole process takes just 15 minutes.

Icon's developers say small areas of decay, or 'caries', can be treated early before they develop into large cavities, sparing patients more invasive treatment and discomfort.

The treatment has been developed by German company DMG Dental Products in conjunction with the University of Kiel and the Charite medical school in Berlin.
